Today, forty years after the founding ofthe Swiss AO group, we have, not the fourth edition of the AO Manual ofInternal Fixation, but a new publication, AO Principles of FractureManagement, divided into six sections and forty-nine chapters. Thistextbook is designed not merely to be a handbook of osteosynthesis, butrather to offer comprehensive recommendations, supported by clinicalguidance, on the principles of how to treat fractures in the newmillennium.

The editors handsomely achieve this goal.The first section covers the philosophy of the AO group, firstdescribed in 1958, relating to anatomy, stability, biology, andmobilization. This section covers the biology and biomechanics offracture management, the materials, and the AO fracture classification.The classification is logical but remains a little complex. Thepathophysiology of soft tissue injury is discussed, with emphasis on theHanover Fracture Scale and the new AO soft-tissue classification,instead of the widely used Gustillo and Anderson classification.The second section deals with the basic principles of preoperative planning for diaphyseal and articular fractures.In part three the reader will find reduction and fixationtechniques.

These techniques (such as nailing, tension bands, and soforth) offer absolute or relative stability. A new concept in thissection is the 'internal fixator with locked screws' thatchanges the original AO philosophy of internal fixation.

Locked screwsare used in newer implants like the PCFix (Point Contact-Fixator) andthe LISS (Less Invasive Stabilization System). They are usuallyunicortical and work more like bolts than screws. They offer theadvantage of very simple handling, allowing safe application byself-drilling and self-tapping. There are 20 well-structured chapters (361 pages) in the fourthsection, which cover specific fractures from the scapula to the foot andspine. This includes everything needed for surgical treatment offractures in a comprehensive and clear way. The reader is presented withuseful information related to specific clinical problems followed byoptions for surgical treatment, pre-operative planning, pitfalls, andcomplications in open reduction and internal fixation and post-operativemanagement.The fifth section is devoted to general topics such as themanagement of open fractures, care of fractures in children, generalpreoperative and postoperative considerations, and polytraumamanagement. This section also focuses on the most important component ofhigh-energy trauma-injury to the soft tissues.The last part contains five chapters that are concerned with themain complications seen in orthopaedic surgery, such as acute andchronic infection, mal-union, nonunion, and algodystrophy.The text is well structured and this book has two additional majorstrengths.

First, the number of quality illustrations, most of which arein color, distinguishes this text from the few others that exist. Thereare also very useful and interesting clinical photographs. The secondstrength of this text is the online bibliography and updates, whichsignificantly enlarges the number of current references.But the highlight of this publication, making it innovative in thepaper-publishing field, is the CD-ROM version that accompanies thistextbook. This electronic version contains the full text of the book,including all the illustrations in (encrypted) Adobe Portable Documentformat (PDF)-the screen version perfectly mirrors the bound one.

Theuser will find dozens of video clips showing reduction maneuvers andtechnical tricks, and will also have access to the bibliography foreasier study. An additional benefit is that the information on the CD iseasy to access and read; there is no need for specialized computerskills nor is there a complex installation program. With the progress ofcomputer technology, I am sure that we will be able to see longer andmore detailed videos in future versions.In summary, I believe that this publication is a remarkableachievement and I strongly recommend this textbook not only toorthopaedic residents but also to practicing orthopaedic surgeons. Thisbook should reside in the libraries of all hospitals.

The majority of this textbook is dedicated to detailing the techniques available for the repair of fractures which it does admirably.The illustrations, which range from simple line drawings to photographs and algorithms, are excellent. When you combine these visual aids with concise and accurate descriptions, the result is a very useful guide for the beginner orthopaedic surgeon whilst at the same time acting as a reference tool for the more experienced operator. The information given on complications, corrective osteotomies and arthrodesis is also very well presented and useful.
